- Couple: $200/month

- Family 5+ Members: $100/month per member
Plan includes: Up to 2 Visits per month for a member without a copay

- Vision Screening
All Plans have a minimum 12-month agreement which includes laboratory blood tests at a 50% discount.
No Vaccinations or Immunizations for children under 18 or patients on Medicaid.
